Childbirth and Postpartum Cardiomyopathy (PPCM)

Pregnancy and the anticipation of new life is a time of excitement for most women. The gestation period is also tinged with some anxiety about the health of the child, and a safe delivery. Most pregnancies are relatively trouble-free, but there are those pregnancies which do not evolve as planned. Take a woman in her late thirties and pregnant for the third time. Previous pregnancies and deliveries were fairly routine and uncomplicated. Until today, this pregnancy was as normal as pregnancies ever are. Labor starts and after 30 some hours the doctor decides that a cesarean delivery is advisable. The operation proceeds normally and within the hour a new baby has arrived. The balance of the hospital stay meets the criteria established for release from the hospital and the new mother and baby go home. Within three days the mother is having problems breathing and relatives start to take her to the emergency room, but en route she cannot breathe at all and it is necessary to call for emergency transportation to complete the trip. The local hospital transfers the patient to a regional hospital that specializes in heart disorders.
The patient has been diagnosed with PPCM (peripartum cardiomyopathy). PPCM is a rare form of dilated cardiomyopathy in which a weakened heart is diagnosed in the final month of pregnancy or within a few months after delivery. Cardiomyopathy is considered peripartum when there is absence of heart disease prior to the pregnancy and no other cause can be found. As with all forms of heart problems the heart cannot effectively support the body and the condition affects the lungs, liver and other body systems. In this country, PPCM is a complication of 1 in every 2,000 - 4,000 deliveries. It can occur in any race, at any age during reproductive years, and during any pregnancy.
Symptoms can include difficulty breathing while lying flat, shortness of breath with exertion, edema, cough, frequent night-time urination, and heart palpitations. Patients as well as health care professionals frequently dismiss symptoms as part of normal pregnancy. Early detection and treatment are extremely important to the PPCM patient. Delays in treatment are associated with increased mortality.
PPCM has no known cause. Researchers are investigating viruses, auto immune system problems, nutrient or mineral deficiencies and genetics as possibilities that contribute to or cause PPCM.
The Internet Journal of Anesthesiology, 2007, Volume 12 Number 1 lists case histories of patients with PPCM and the treatments that were used for these patients. The underlying theme of this and other research is that both the patients and health care providers need to be more aware of the symptoms. Patients need to be persistent when seeking treatment if they exhibit any of the symptoms usually associated with PPCM.
In 2000 the National Heart, Lung, and Blood Institute and Office of Rare Disease (National Institutes of Health) Workshop Recommendations and Review concluded: "Peripartum cardiomyopathy is a rare lethal disease about which little is known. Diagnosis is confined to a narrow period and requires echocardiograph evidence of left ventricular systolic dysfunction. Symptomatic patients should receive standard therapy for heart failure, managed by a multidisciplinary team. If subsequent pregnancies occur, they should be managed in collaboration with a high-risk perinatal center. Systematic data collection is required to answer important questions about incidence, treatment, and prognosis. "

Breastfeeding for Smarter Babies




For years, new parents have been hearing that "breast is best," and each decade, more and more women choose lactation as the primary food source for their newborns. It appears that breastfeeding may now produce a generation of smarter, happier babies. Studies have shown that babies need touching and nurturing to develop and survive and breastfeeding supplies both needs on a regular basis.
Breastfed babies followed from birth to 6 years had higher IQ scores than formula-fed babies. This is not a new finding. Past research has shown that mothers from more affluent backgrounds were more likely to breastfeed. Consideration was given to the fact that improved mental abilities may have been related to family circumstance as much as breastfeeding.
Researchers from Canada's McGill University attempted to overcome the possible influence of family economics by evaluating children born in hospitals in Belarus. The group studied over 13,000 breastfed babies born in 31 maternity hospitals. Some of these hospitals ran breastfeeding promotions to boost rates across all groups. Some provided nursing training and provided support for breastfeeding mothers. The mothers who received the training and continuing support were more likely to nurse for a longer period of time.
The children were divided into groups for evaluation depending on whether their mothers were given nursing training or not. Babies who were exclusively breastfed for the first three months scored 5.9% higher on IQ tests in childhood. Tests indicated that the longer the babies were breastfed the more significant the intelligence difference.
When these children began school teachers also gave them significantly higher academic ratings in both reading and writing than children in control groups. The Archives of General Psychiatry lead researcher Professor Michael Kramer said, "Long-term, exclusive breastfeeding appears to improve children's cognitive development." Professor Kramer also said that it was not known if the increased intellectual development was due to some nutritive value of breast milk, or related to the physical and social interactions of breastfeeding.
There are several reasons that breastfeeding may improve the mental development of babies:
Breast milk contains fatty acids and other nutrients that are necessary for the development of babies.
Physical and emotional aspects of breastfeeding may lead to permanent improvements in brain development.
Breastfeeding may increase verbal interaction between mother and child which could aid development.
Though the exact mechanism of improved intelligence as a result of breast feeding is not known there are also other reasons for breastfeeding; children who are breastfed generally have fewer gastrointestinal problems and they have better protection against obesity, diabetes and cancer. Women who breastfeed have a quicker recovery from childbirth and breastfeeding reduces a women's risk of developing breast cancer.
Breastfeeding is natural and good for the baby and the mother. A pregnant woman who wants to breastfeed but isn't sure how to began can ask her physician or midwife for a referral to a professional, or any of the many groups who advocate breastfeeding.

What Is Kindness To Parents?



The parents are entitled by right to kind and dutiful treatment from their children. Since this is an important duty that Allah emphasized so strongly, it is essential for every human being to know what constitutes kind treatment of parents. It is no exaggeration to say that for a believer, to be a dutiful son or daughter is to take the way that surely leads to heaven. We note first that Islam uses the Arabic word birr in connection with children's attitude towards their parents. The term connotes kindness, compassion, benevolence, and almost every aspect of good and generous treatment of others. One of Allah's own attributes is derived from this root. Allah is the "Barr," which means that His kindness, compassion, grace, and generosity never fail. Scholars say that this term includes everything that is good. Muslim scholars divide birr into two main branches; financial and non-financial. In respect of children-parent relationship, if either or both parents are poor, children must support them according to their means. This is not a matter of choice. Islam makes it a duty on the children to look after their parents, providing them with the same standard of living as they provide for their own children. If they are well off, to go beyond the mere provision of what is necessary for a decent living, so as to allow their parents to share in the comforts and luxuries that they can afford, is to make an investment for the hereafter. Nothing goes amiss with Allah. Allah is pleased with any son and daughter who please their parents. Looking for Allah's reward, some people make their parents feel that whatever they own is their parents' as well. They can use it in the way they please. Although some people are careless how they spend their money, most parents are more careful when it comes to spending their children's money than spending their own. So, to make one's parents feel that they do not live on their children's charity is to give them that kind of trust that makes the difference between feeling oneself to be a burden and feeling perfectly at home. The more parent feels happy and contented with their children, the more Allah is pleased with those children. Moreover, parents pay their children back immediately. This takes the form of praying Allah for them. Such a prayer by parents for their children, which for Muslims, normally takes the form of "May Allah be pleased with you," is certain to be answered. When Allah is pleased with someone, He helps him or her overcome their difficulties, eases their hardships, and guides them to success in this life as well as in the hereafter. The duty required of sons with respect to financial support of their parents is to provide them with what is reasonable according to their means. A son of moderate means cannot be expected to provide his parents with the same standard of living as a much wealthier son. Although we speak of this as kind treatment by children, it is indeed a repayment of a debt. Parents look after their children when they are young and helpless. They provide them with all they need as much as they can. Moreover, they do it willingly. Children take what they are given unaware of how much effort their parents exert in order to earn money for them. When the children grow up and their parents are in need of their support, that support must come naturally, without letting the parents feel themselves to be a burden on their children. Apart from financial support, children must respect and honor their parents and extend to them the sort of treatment that befits their status as parents. In any social occasion, and even when they go out with their parents on the street, children must not precede their parents or take a higher or more favorable position than theirs. Children should always allow their parents to take precedence. In Muslim societies, that sort of treatment always earns children more respect. Muslim society looks down on anyone who do not extend to their parents the standard of honorable treatment expected from children. Moreover, children are expected to do as their parents tell them. From the Islamic point of view, this does not apply only when a child is young. As long as a son or a daughter is able to grant the wishes of their parents, and by doing so they neither incur any sin, nor jeopardize any greater interest, then they should do so as if these wishes of their parents were commands. There is nothing excessive in this. It does not impose a great, heavy burden. Normally, a parent is easy to please. Even when parents ask for something that is difficult to obtain, children can maneuver their way to please their parents without undertaking any great difficulty. Some parents may be unreasonable in their demands, especially when they live with their son in the same house. Relations between his wife and his mother may be occasionally strained. A mother may feel that her daughter-in-law takes her son away from her. That may lead to friction between the two. A wise son tries his best to reconcile his mother's rights with those of his wife. He must not be unfair to either. Should his mother ask him to divorce his wife, he must not do so if his wife fulfills her duties toward him and his mother. All that a daughter-in-law is required to do towards her mother-in-law is to look after her in a reasonable manner. Even in such kind treatment, children are only paying back a debt to their parents. No matter how great a burden the children bear, they do not pay them back adequately. It is very rare that a parent is so ill and handicapped that he or she needs to be looked after in the same way as a baby is looked after by his parents. `Abdullah ibn `Umar, a leading scholar among the Prophet's Companions once saw a man from Yemen carrying his mother on his back and going around the Ka`bah in his Tawaf. Rather than show any sign of complaint, the man was happy, repeating a line of poetry in which he likened himself to a camel his mother was mounting. The only difference is that a camel may be scared by something and go out of control. He would never go out of her control. He looked at `Abdullah ibn `Umar and asked him whether by so doing he discharged his debt to his mother. Ibn `Umar said, "No. You have not even paid back one twinge of her labor pain when she gave birth to you." (Al-Bukhari in Al-Adab Al-Mufrad and authenticated by Al-albani) That was not an exaggeration by Ibn `Umar. The Prophet (peace and blessings of Allah be upon him) defines the only way through which children repay their parents fully. He said, as related by Al-Bukahri in his book Al-Adab Al-Mufrad and by Muslim and others on the authority of Abu Hurairah, "No child repays his parent fully unless he finds him a slave, then he buys him and sets him free."

Severe Stress In Pregnancy Linked To Schizophrenia

A new study has revealed that if pregnant women endure severe psychological stress, like living in a war zone, during their early months of pregnancy, their child is more likely to develop schizophrenia. The lead author of the study, Dolores Malaspina M.D said, "The stresses in question are those that would be experienced in a natural disaster such as an earthquake or hurricane, a terrorist attack, or a sudden bereavement." The study examined 88,829 children who were born in Jerusalem between the years 1964-1976. Babies born to mothers who were in the early stages of pregnancy during the 1967 War had much higher incidences of schizophrenia when compared with the rest of the population. "A developing foetus requires some exposure to maternal stress hormones as it normalizes their stress functioning. But women experiencing anxiety or excessive stress would do well to address it before a planned pregnancy and to have good social support systems," Malaspina concluded from the study.

Amerigroup Will Pay $225M To Settle Allegations That It Denied Coverage Of Pregnant, Sick Illinois Medicaid Beneficiaries

Amerigroup will pay $225 million to settle allegations by federal and Illinois state authorities that Amerigroup's health plans denied coverage to pregnant women and people with health ailments in the Illinois Medicaid program, the insurer and the U.S. Department of Justice said on Thursday, Reuters reports (Dixon et al., Reuters, 8/14).

Cleveland Tyson, a former vice president of government relations at Amerigroup's Illinois subsidiary, in 2002 filed a whistle-blower lawsuit that claimed the company cherry-picked the healthiest patients to reduce spending. In March 2007, a federal judge in Chicago ruled against Amerigroup and awarded $334 million in the lawsuit.

Amerigroup, which first announced the proposed $225 million settlement agreement in July, said that it also will pay $9 million in legal fees in the settlement, and the company has agreed to enter into a corporate integrity agreement as part of the settlement (Kaiser Daily Health Policy Report, 7/23). DOJ also said that the insurer's appeal of the $334 million court judgment will be dismissed as part of the settlement.

Amerigroup last month announced a second-quarter loss of $162.5 million on a one-time, after-tax charge of $199 million for the settlement and said that it will pay for the settlement from an already established fund.

In a statement, Patrick Fitzgerald, the U.S. attorney for the northern district of Illinois, said, "A settlement of this magnitude sends the clear message that this office takes health care fraud very seriously," adding, "This case also illustrated the perils a defendant faces in taking a case such as this to trial." Amerigroup said it agreed to the settlement to reduce the legal and financial uncertainty for the company. In a statement, Amerigroup Chair and CEO James Carlson said, "Our responsibility to our shareholders, associates, members, providers and government partners is to close this chapter now and move toward the future" (Reuters, 8/14).

Morning sickness affects most women at some point during pregnancy, with symptoms ranging from mild nausea through to full on sickness, which can be quite debilitating. It can also occur at any time, not as the name suggests, only in the morning. There are some things you can do to help yourself however. Try sipping peppermint tea when you feel nauseous, and eat little and often throughout the day. Some women also find that eating something, such as a piece of dry toast or a plain biscuit before they get out of bed in the morning can help. Pregnancy: The Nesting Syndrome

Women everywhere who have been pregnant have felt the nesting syndrome take over their body. The nesting syndrome can happen anytime during a pregnancy but often happens between the 5th and 9th month of gestation. It is a primal instinct but many pregnant women may look a little crazy when they get swept up in the syndrome. Nesting varies from woman to woman just as pregnancy varies in each woman. Some women feel the syndrome mildly and others suffer from an overwhelming case of the nesting syndrome.
Nesting is both biological and emotional. A part of it is instinct. A woman knows her body best. Women seem to know when the baby is about to arrive and thus the nesting syndrome takes over. Pregnant women can be found at this point cleaning vigorously, rearranging the baby's room and packing and unpacking the labor bag.
Wild animals as well as domestic animals also have a nest it instinct. That is why you will see a mother bird adjusting the weaves of her nest, a mother bear storing food and a mother cat finding a hidden area of the home where she will birth her babies. Pregnant women are just like the mothers of the animal kingdom getting ready for their offspring.
Emotions come into play with the nesting syndrome when pregnant women want to prepare. A woman's baby deserves the cleanest, healthiest atmosphere and that is a mom's job. Cleaning, organizing and being prepared are all elements that come into play when a pregnant woman experiences the nesting syndrome. At times the increased energy and overpowering need for pregnant women to follow through with these things before birthing their child can lead to an emotional crisis. Often these crisis' are short lived because there is often something else to be done.

Pregnant women who experience the nesting syndrome should keep a few things in mind. Read the labels of your cleaning products before cleaning. Cleaning products should be safe to handle and should be handled with proper care. Use environmentally safe brands that use little to know chemicals. If you must use chemicals then put on rubber gloves, a face mask and only use in well ventilated areas. Open up the windows and put on a fan to circulate the air. Pregnant women should avoid cleaning out the cat litter box because it can lead to a problem called Toxoplasmosis, this is a parasite found in a cat's fecal matter, and if transferred to the unborn child it can cause severe harm. The complications that arise from toxoplasmosis include but are not limited to blindness, deafness and mental development disorders as well as other health complications. Pregnant women need to stay away from the cat litter box.
The nesting syndrome is something that just has to be dealt with by pregnant women and often their husbands too. It can be an amusing time as well as a frustrating time. It is worth it in the end when you bring your bundle of joy home to your clean comfortable well organized home.

Ovulation is very important when trying to conceive. After all, without the egg, there can be no baby. Knowing when you ovulate is one of the most important factors in getting pregnant. There are a lot of different methods of finding out your ovulation time. Here I am going to briefly cover a few of those methods.A simple way to know when you ovulate is to use an ovulation predictor kit, otherwise known as an OPK. An OPK works like a pregnancy test in that it will most of the time give you one line for a negative and two lines for a positive, but there are some key differences. An OPK will often times give you two lines, and one will be much lighter then the other. This does not always mean that you are ovulating. Be sure and read the instructions that come in OPK's before using or interpreting them. OPK's will tell you when the ovulation hormone is in your system. This is useful because the OPK is used to predict that you will be ovulating soon. To know that you already ovulated would be to late to have intercourse in order to get pregnant.A little bit more complex way of checking for ovulation is checking your cervical mucus. This can be done by simply wiping a piece of toilet paper across the opening of your vagina and seeing what is on the tissue. Cervical mucus changes a lot throughout your cycles. Usually, right after you end your period, the cervical mucus will not be abundant and will be sticky. As you get closer to ovulation, that cervical mucus will thin out and eventually resemble egg whites. This fertile cervical mucus allows the sperm to survive longer in the usually harsh and acidic vaginal environment while waiting for an egg to be released. After you ovulate, your cervical mucus will begin to dry up and, if not pregnant, you will eventually start your next period.

Another way to check for ovulation is to simply check your calendar as well as your body. If you have a regular cycle, meaning it is the same length each month, checking the calendar is a great way to detect when you ovulate. All you do is check the last day of your cycle, and count backward fourteen days. That will most likely be the time that you ovulate. So when you come to your next cycle, you will need to have intercourse on the few days preceding your ovulation in order to increase your chances of becoming pregnant. Also, around your ovulation time, some women will experience some slight pain, known as ovulation pain. This varies from woman to woman.Ovulating is a complicated thing but once you become familiar with your body and the signs it gives you each month, you should be able to track it fairly regularly. Nobody will know your body better than you, so pay attention and before long you will be able to detect your ovulation without any problems!
